Referrals

What Happens After We Receive a Referral?

1. Initial Contact

Our team contacts the individual, family, Support Coordinator, or Care Coordinator to discuss the requested service, approved hours, schedule, location, and support needs.


2. Intake Meeting

We meet with the individual and family to learn about the person’s routines, communication style, preferences, strengths, medical needs, safety concerns, behavioral supports, goals, and expectations.


3. Document Review

We review the Individual Support Plan, service authorization, assessments, medical information, medication list, emergency contacts, risk-management information, and other required documents.


4. Staff Matching

We identify a qualified staff member whose availability, experience, personality, and skills are appropriate for the individual’s needs and preferences.


5. Introduction and Training

The staff member is introduced to the individual and family and trained on the person’s support plan, routines, goals, safety needs, communication methods, and documentation requirements.


6. Services Begin

A start date is coordinated after the required documents, authorization, staffing, and training are completed.

What Families Should Have Available

To help prevent delays, families should be prepared to provide:

  • Medicaid and insurance information 
  • Current medication list 
  • Medical and diagnostic information 
  • Emergency contacts 
  • Allergies and dietary restrictions 
  • Preferred service schedule 
  • Information about routines, interests, communication, and support preferences

Make a Referral

A&E Noble Care Agency welcomes referrals from individuals, families, Support Coordinators, Care Coordinators, healthcare professionals, schools, and community partners. Please provide as much information as possible so our team can determine whether we can meet the individual’s needs.
